Once your application has been accepted and your team has been allocated a place, please complete the form below.  The information provided will be used to compile the league address book for season 2026/27and will be shared with your opposition so please ensure your form is completed accurately.

Please complete one form per team entered.  Forms will not be accepted with multiple team information included.

Therefore, if you have two or more teams you will need to complete one for each team.
